A landlocked parcel has no legal right of access to a public road. You may well be able to drive to it, across a neighbour's field or down a track that has been used for decades, but if that route is not a recorded easement, it is permission rather than a right, and permission can be withdrawn.
That distinction is the whole problem. A handshake with the neighbour works fine until the neighbour sells, dies, or changes their mind, and it does not survive a title search. Buyers find out during due diligence, lenders refuse to fund, and the deal dies late.
Almost always by division. A larger tract with road frontage gets split, the frontage portion sells first, and the back half is left behind with nothing formally reserved for it. Do that across two or three generations of family splits and the interior parcel is stranded.
